Complete Is this an IA?

A few years ago I turned in a friends lab report instead of mine on accident as I forgot I had theirs on my computer. Their's was from a few semesters prior. My professor noticed it first and said this was a conduct violation to have theirs on my computer. She asked me to re-upload my own lab report and find a time to meet with her. I responded offering to meet as soon as possible, and turned in my own lab report. I never heard from her again, never met with her, and the next week ended up receiving my lab report back, getting a decent/ normal grade (no markdown due to the event). I finished the class without my TA or prof ever mentioning it again, and got a passing grade. I am wondering if this is an IA, or how I would find out if this is an IA? If it is not, do I still need to report this?

I have ordered and looked at my official transcript, and it is not mentioned, it only includes my courses, grades and degree. Past this, I am now trying to figure out if there is another official record where this incident would show up, such as a disciplinary record. However, my university's page states we should have received an email if something was filed against us, and I did not ever get one.

Should I report the incident regardless of whether it was a formal IA or not? Other threads show that consensus seems to be that being upfront is better than taking the risk of not and having someone find out later.

1) Some schools do record these things in other records. Though it doesn't sound like this resulted in an IA, call the Dean's Office and tell them you want to be sure you have no Institutional Action on your record. Also ask if there is any other office that might keep such records.

2) Could be a new policy, instituted after the time of the event.

3) No. Report it only if, through due diligence, your college confirms it exists.

Sounds like you had a warning. You would KNOW if you had an institutional action because you would have had followup with an administrator about it. In this day-and-age with cloud drives and collaborative writing tools, this can be considered an honest mistake to load the wrong file. Don't be sloppy the next time. No you don't have to report this, but if it comes up in an interview about soliciting a story about attention to detail or a mistake you learned from, file this one.
