last-minute ROL advice

I'm going into peds and having a last-minute ROL dilemma. I finally have my top 3 settled, and am pretty comfortable with the order of the bottom 5, but I am having some issue with my fourth and fifth choices as to which I would rather be at. Advice would be appreciated.

Program A is a middle of the road program academically in a smaller city. It has few fellowships, which is a strength in terms of resident autonomy but potentially a negative for exposure to subspecialties and future fellowship opportunities (I am not sure if I will pursue fellowship, but want to keep my options open). I really liked the advocacy and international opportunities, the PD seems awesome, and got a good feeling overall from the residents and program in general. It seems like a more friendly and laid-back environment. I can "see" myself easier in this program, but I am concerned if I would be happy with the location and if my fiance would be able to find work in his field there, which is a major quality of life issue. Also, board pass rates are below average which is concerning to me.

Program B is a top 20 program in a larger city. They have good exposure to subspecialties and many fellowships, although do not appear to be fellow-driven, and great board pass rates. Much less emphasis on advocacy & international opportunities, although these are possible if you are willing to set them up more yourself. My fiance would definitely have better job opportunities there. I did not get a particularly good or bad feeling there, but it does seem that this program is more intense and residents may be more stressed out although still happy overall. I feel better about the academics here but worry that I won't have the time/energy to pursue my passion for advocacy work.

Bottom line, I definitely feel like the residency program at A would be a better "fit" for my personality & interests, but B offers a better location and stronger academics with less concern about how my & my fiance's career opportunities will be affected. All of these things are pretty important so I am having trouble deciding how to rank these! My top 3 are all pretty competitive places so it is reasonable to think I could end up at my 4th or 5th choice. What would you do?

Go with program A. Seriously it probably doesn't matter anyway. Peds is not competitive and you will probably get your top3 anyway.
 
Just follow your heart. That's what I do.

I disagree. Go with B: a happy spouse means a happy resident.

At first I was going to say that you should go with A. But the more I think about it, the more I agree w/ Samoa. You're going to be busy and stressed out as an intern no matter where you go, it's just the way it goes. If your spouse is also stressed out b/c he can't find a job or hates the city you moved to then you'll be doubly miserable. It took my wife 5 mos to find a job she liked when we moved to Portland for internship and this made both of our lives more difficult.
 
Definitely B due to improved job opportunities for your spouse and still a great program for you.
 
Program B. Your outside interests and advocacy work are great, but residency is primarily for residency - to learn to be a great doc. You can do everything else afterwards, and great clinical experience and academics are what matter now. All that being said, plenty of people work on projects and outside interests during residency, regardless of their program - just because it's not as common in Program B as in A doesn't mean you can't do it.
 
Thanks all for the advice...after much deliberating and soul-searching, I decided to go with Program A for #4 and B for #5. I know B is the more logical choice to rank higher, but I haven't had a lot of gut feelings in this process and I know in my heart if I don't get one of my top 3 I will be happier at A then B, even if it is not as strong academically. I just really liked the people at A and felt like they really cared about and supported their residents, a feeling I did not get as much at B; and while it's not a top program it is solid academically and people have gotten into good fellowships from there in recent years. I talked (at length!) about this choice with my fiance, he is in a field where he can do freelance work conducted primarily via internet which will help and he should eventually be able to find work in his field in that city even if it is not his dream job, so he is okay with it. I am still scared and a bit confused, but hopefully I will get one of my top 3 choices and it won't matter anyway! :xf: Good luck to everyone.
 
May however you end up ranking your 4th and 5th choices be made completely moot.
