take it like this CS is like a thesis, every case you want to prove or disprove your most likely diagnosis (thesis) to do so is by supporting it with the most prominent positives that made u think about diagnosis X and the lack of symptoms / signs of disease Y makes diagnosis X most likely so for example 40 Yr old Female g4p4 presented with RUQ pain less than 6 hrs , post food + Nausea +Vomiting your thoughts would go with multiple choices such as gallbladder stone, cholecystitis, cholangitis etc
to prove your diagnosis you should support it with the findings so:
1- gallbladder stone
RUQ pain less than 6 hrs, post food
age 40
female
fertile
nausea
vomiting
the above are the positives
lack of fever (thats the negative that I had in mind)
so answering your question you only list that support your diagnosis.

dont recheck the Bp u barely have time
