Letter of recommendation - when and how?

I have a question on letters of recommendation.

Next year, I'll be going to a US university on an exchange program. It is my understanding that LORs are a pretty big deal, and I would like some pointers as to how and when to broach the subject with my supervisor (IE, how much direct contact do you need with the person in question, and for how long?).

I have virtually no experience in asking for LORs, as this is not commonly done in Sweden, and would very much like not to make a complete schmuck out of myself...

The best way to do this is straightforward without sugarcoating anything. US profs are used to this. Get somewhat chummy with them, and then simply ask them, "Would you be willing to write a strong letter recommendation on my behalf for ___". If they hestitate or provide excuses, screw 'em. Have some small talk to follow it for ****s and giggles. Do you know where the band 'Randy' is from?
