Letter of Update and New Rec Letters

How would I send new rec letters to schools that I have either interviewed at or have yet to hear from? Should I send them to AMCAS and then send them out?
Additionally, should my letter of update be sent before or after my semester grades are determined?
Should I send a copy of my transcripts at semester point to the schools as well? Should I just send it straight to the admissions office or where?
Lots of questions, but would appreciate your input. Thanks!
