LLU School of Dental proof of shadowing form?

Does LLU have an official form they want the applicant to fill out and have signed by their dentist as proof of hours?

I can't find that form anywhere, but remember seeing that they wanted it

I saw it when I was initially looking around their website to see if I wanted to apply there. I saved it on my computer. But on their secondary I can't find anywhere that they say they want it. I'm guessing that is an old page they still have on their website, so I'm not going to worry about it!

I just called the office and they were really helpful. The form can be found on their SOD website

LLU SOD --> admissions and recruitment --> Dentistry --> scroll down until you see "forms and additional information" --> observation/shadowing hours. viola!

I think students have an option of adding "additional recommendors" on the supplemental. I am not going to have my professors resubmit another form (because that would be too much of a hassle for them) but for my spiritual recommendor, I will certainly have her fill one out.

I'm thinking if I submit it electronically, it asks the recommendor for all that information so there wouldn't be a need to submit that form unless they were doing it via snail mail. what do you think emurocker?

Try to have some SDAs writing you the LORs or filling out the forms.

I think your game plan sounds good... I was thinking the additional form would be a hassle for my non-spiritual recommendors too. I think I might just call them on Monday and clear it up. I'll let you know what I find out!

Even though it doesn't say to send this you still have too. I called in today and asked them if they needed anything else after the secondary like transcripts or official dat scores and they said the only final thing after the secondary app is this observation form so make sure you send it! Thanks shiftypoptart! I shadowed several specialists and general practitioners, but am only going to have one of them sign for my 20 hours. Is it necessary that I have someone to vouch for all the hours I've logged?

I'm not quite sure. I have the 2 science professors, 1 general dentist, 1 research/professional and the 1 spiritual recommendor. Do you think we'd need 6 recommendations?

Ok, here's the story:
-For the reference form, you can either have your recommendor write a letter or fill out a form, not both. I was thinking about trying to get all 6, but decided to stick with three (science prof, employer, spiritual) + another science prof who was supposed to get his letter in by July...but I've been steadily harassing him more so hopefully he doesn't leave me hanging!!!

-Shadowing form-you need one for every office you shadowed. One form per office.

As far as the alcohol/tobacco/drug question goes, I'm going to answer it honestly, for better or worse.
