LoR from a professor I haven't kept in contact with?

Basically I had a biology professor about a year ago who I was very close with and took many classes with along and also did field work with. However, I transferred out of that college and haven't kept contact her since then. Would it be a good idea to ask her for a LoR and what would be the best way to go about it?

I would go to their office to say "Hi". Professors like to know what former students are doing. Your professor should remember you if you took several classes with them. Share with them that you'll be applying and if they can provide a strong letter of recommendation. They'll probably say they will be glad. At that point you can give them instructions on how to complete a LoR.
 
Of course you should ask. They knew you well at one point! Just do a little catching up first.
