loyola secondary help

hey

anyone know what the loyola secondary Evaluation Inventory Form is and/or if it's available online?

(I'm out of the country right now, and only know that I have recieved a loyola secondary with due date via snail mail at my home (usa) address, but i don't know if there was an Evaluation inventory form with it).

The form came with the secondary invitation through snail mail, and unfortunately, I do not think that it is available online. Hope that helps.

if you go to that site, you can access the questions for the secondary. what you need to turn in is the printed copy of the secondary (from online), the 'evaluation inventory form' and the $70 fee.
their phone number is 708.216.3229 (Thursdays, 9am - 5pm).. you might want to call and see if you can send the two papers separately?
