Man that test was ridiculous

did anyone recently take the Family Practice in service training examination? I did all of the old questions at least once from the past few years, and this one was BY FAR AND LARGE much much tougher than any one of the other tests that I have ever seen, especially the true and false section, which was just ridiculously insane, would you order this test and this test and this test for this problem, true or false... damnit... It sure didnt help that I had to freakin round on six patients before the examination begun at 7:30, up since 4 am.. not to mention annoying as hell and stupid nurses calls from the floor during my ONLY lunch break during the examination, 400 or so brutal questions, only a few were basic and really simple I thought, anyone else feel this same way???? and what makes me feel even ten times worse, is that traditionally my residency program scores very high scores on this examination, then why do they still make you round on patients in the morning if they want for you to do well? man..... tough... 😡 even though it doesnt count for sh+t, I am still pissed off that I dont think that I did so well.....
 
All I have to say is, what the hell was that black sh+t in that guy's eyes? Man that fundoscopy stumped me. All I know is if I ever see some dude with some f*ck up fundo like that he'd be out of my office ASAP to ophtho, I don't care if he's blind.

yeah man and that EKG was so f+cked up, I aced ACLS course, and couldnt make heads or tails of that one for the life of me, is it a left anterior hemiblock, a left anterior fascicular block, right bundle branch block, partial left hemiblock, what the hell man???? man that was just the most ridiculous test..... made USMLE look easy...
 
Do all specialties have in-service exams?

In-service exams are not the boards, right?

Do all the residents in that specialty take it at the same time (much like the MCATs)?

hey, yeah I think that thats the case, but Family Practice has this examination annually with all residents R1 to R3, just to see where the residency programs each stand nationally, and the program directors of each program get the results of their own residents. Its not necessarily the boards, boards are pass or fail, these are just see where you are and how much you have progressed type of boards - damn we really need a Starbucks nearby the hospitals and clinic, that would be extremely nice and convenient, as I am sick and tired of regular coffee, does the job but I miss my latees.. I suppose that everyone in this specialty take this examination at the same time..... just sad that we have to work on our patients before we take the examination, day was just freakin ridiculous, morning at 445 am round on all my patients before the test at 730, lunch 'break' annoying nurses from the floors bugging me about stuff, go back to the test, and then discharge two patients on the floor after I the test.... man discharge dictations, discharge dictations, rounds, etc on top of this ridiculously hard test. 😡 could I picture myself doing anything else though right now? probably not.

Dude, I'm pretty sure it was WPW. I dunno. So what's the deal at your program regarding the in-training exam? It doesn't "count" here, but they do have a minimum cut score and if you don't well, faculty assigns you CME courses to take.

you know any type of wording would have been helpful in that EKG case, "a 12 year old kid with recurrent episodes of syncope or dyspnea" and then I would have shot for WPW, I was looking up and down that EKG for some Delta waves incidentally, but couldnt find them clearly.... so you were probably correct I mean it sure didnt look like any block to me for sure, damn either way that was a cardiologist question, ridiculous, or they should fking schedule this test much much after we gain more clinical experience, say like March or April, so ridiculous man. Dude theres a cutoff there? that fkin blows, I have heard nothing of this sort, but that cutoff must be pretty low then hopefully, like 60% or maybe even 50% lol is that even close to reality probably?, hell no.. but that would be my ideal cutoff... man CME courses I have heard nothing of the sort, well then I suppose it becomes obvious who didnt do so well, which makes things even ten times worse... what a horrible thing to do to residents, who are already stressed out enough. 🙁
 
i hear ya comrads! what a mess. i think the vignettes were much longer this year than last and what the F***? how much ob/gyn can they possibly ask? i was smoking at the end to try and finish as time was an issue with me this yr and was not last yr. but, lets face it...thats fp for ya... really and truly have to know EVERYTHING! i thought the trauma case was cool. that guy must of had a shattered spleen huh? to the OR baby knife in hand!

i wouldnt worry too much about the test itself. i think it scares us into keeping up with our journals and trying to read more, but the % to pass is pretty low. I heard from other senior residents that just finished and took the 'big one' said they did fine and you should too, if your inservice scores are above 40% on each section. FYI...

p.s. i just knew that kid's xray was paget's, but DAMN me for changing the answer...some people never learn! 😡

oh ok so the big one counts.... damn man... thats just life for us right now though man... Ob blows as a subject (hopefully please somebody dont take that negativley, I actually like IM and Surg questions, so someone can retaliate if they truly feel the need to do so) and sucks big time when they ask ridiculously detailed questions like this.
yeah shattered spleen, intercostals 9 through 11... but I dont think that was in the answer choices, I recall looking for that one too...but they asked something else funky as hell... damn does there actually exist a pass percentage for this test? oh man.... oh man oh man oh man..
 
I thought there were delta waves...WPW in my book. As for the eye...hemorrhage?? I screwed up the timing with my exam. Circled all the answers in the book and then did the bubble sheet...bad strategy. I missed like 20 questions on the bubble sheet...guess it will make next year's exam look all that better. Sigh...
 
Took it post-night float...a very painful experience. I kept dozing off through the exam. I agree that it was ridiculous. The pictures killed me.

I'm sure I'll be called in to meet with my program director when our scores come back...

Yes, all specialties have inservice exams every year. It's the same exam for all the years of the residency program. It's a tool the programs use to gauge your progress in the program. One of their many ways of evaluating if you're progressing in the program.
