My understanding is that after this date, you can hold an acceptance to one school only. But what if I'm still on the waitlist of a school I really want to attend? Surely they don't expect me to give up my spot, right? Can anyone provide any insight into this matter?

It means simply what you said. A waitlist spot is not an acceptance. You may hold as many of those as you wish past May 15th until, of course, they become acceptances. At that point, if you take the new acceptance, you must turn down the one you decided to hold on May 15th.
