I am in the process of submitting my AMCAS and am interesting in pursuing an MPH as well.

What does checking off "MD/MPH" on the AMCAS do?

From what I have seen for most schools, you apply to the MPH after 1st of 2nd year.

Also, if I were to check off "MD/MPH" is is binding? - aside from GW.

List of 4 years programs:
UPenn
Duke
Univ. of Rochester
Yale
Mt. Sinai

Indicating your interest should not be binding, especially since you apply to th MD & MPH programs separately (i.e., you get into medical school first, and then apply to the MPH program separately).

For more information on MD/MPH programs, search through this forum and type in "MD/MPH". You'll find a lot of great information in them.

So, does clicking "MD/MPH" on AMCAS start the MPH application process? or is it only to show interest?

perhaps I should just call all the schools....

No, you would apply to a specific program after you are accepted to medical school.
