I'm headed back into 3rd year of med school this year and am looking ahead into who I should be getting letters from. My current plan is the chair and another ENT but should I also have two separate letters from my comentors? Should they do some kind of combined letter? Also, one of my mentors is world renowned in my field of research and the other less so but had more time to mentor me and get to know me, so if I had to choose one of them, it would be very difficult.

I would get a combined letter from your co-PI's and have one of them submit to ERAS. A letter from your dissertation advisors describing your research and academic potential is important, but I got the sense programs want to hear from ENT faculty they know and who are in a position to evaluate whether or not you'll be a good resident.
