Last year there was no essay or form to fill out and send in. If I remember correctly, they send you a link (or password?) to a website, you fill in some biographical and course info, and that's it. This was by far the easiest secondary I filled out last year.

Yeah, that's it. I'd send you a pdf if there were one, but its all online forms that I can't get back to now that I've submitted mine. It takes like 15 minutes... Basic biographical stuff, LOR information and pay.

Don't sweat it, they'll email you a link and a password, and you'll have it in within the hour.
