I'll forgo spouting the usual caveats of "premeds should not pre-select their specialty," because your situation is unique.

Go to a medical school which has a top Rad Onc residency program. I would say in no particular order: Harvard, Yale, Michigan, Wisconsin, UCSF (a bit of a caveat here b/c administration pushes primary care pretty hard), Johns Hopkins, Penn, Wash U, Chicago, Stanford, etc. As you can see, med schools with the top Rad Onc programs tend to be the top med schools so you will have your work cut out for you. If you go to one of these schools, you can do research in Rad Onc early, identify mentors early, and generally have a strong base for your future residency application.

MDACC and MSKCC don't technically have medical schools but do affiliate with Baylor (? UT-Houston) and Cornell (? Columbia).
