I'm wondering if you have to reveal your medical records while applying for residencies. If you do have to reveal your complete medical history, can this affect you in a negative way? Can this affect your ability to get a liscense as well?

Your medical records are your own business, and medical school do not ask for them. You may get the standard disclaimer about whether you have any medical conditions that would prevent you from fulfilling your role as physician. This question may appear on med school, residency, and job applications in the future. If the answer is a "yes", they may ask you to elaborate.
 
In the state of New York, you are required by law to have a physical exam, TB test and urine drug screen. You don't necessarily submit records or blood work, but they will find out about your heart murmur.
