Medicare improperly paid hospitals millions for IMRT

I'm too lazy to read this - but my understanding is that 77290 is now bundled with IMRT (a change that maybe occurred in 2016 ?). Apparently their software was not designed to catch this after the change ? Interestingly, CMS then said that the simulation is part of IMRT planning and therefore should not have been paid in the past either. Then the OIG tried to recapture that money too.
 
ASTRO just mentioned this in their 'ASTROgram'. They mention "In addition to raising these concerns with the OIG and CMS, ASTRO has also issued coding guidance on the topic to ensure proper billing" and link to a website on this - stating that the changes were effective 1/1/16. CMS/OIG is saying they were effective 20113-2015.

Bottom line- it looks like the overpayment was due to CMS being unclear on this (until 2016)
