Young men everyday head to their local military recruiter with visions of becoming a Navy Seal, Marine Commando, Air Force pilot or Army Green Beret. They are suckered in with promises that all their wildest dreams will come true. The reality is for every Seal, the Navy has 10,000 utter crap jobs, for every fighter pilot, they have 200 guys who sole job it is to clean up the toliets in pilots' quarters.
Medicine is the same exact thing, most of the MD jobs suck big time. People will try to convince you that doing primary care is your wildest dream...no that is latrine duty. Dont listen to so-called mentors in medical academics who say you would be a great toliet cleaner..err, I mean internist, they are the military recruiters of this profession. They will lie, cheat and steal to make their quotas of suckers.
The solutions is simple, everyone do radiology and derm. They are by far the most choice fields, in fact I would go so far as to say even surgeons are radiologists' "little beatches" from the lifestyle perspective. Then once medical schools are faced with 100 people who want to rads and derm and no one who wants to do Ob, general surgery or pediatrics maybe things will change.
Realizing that likely wont happen, another approach would be for all residents and attendings demand only they review radiology and never get a derm consult. In fact, surgeons should completely take over derm. Screw them.
