Medullary cystic disease and Medullar sponge kidney...how to differentiate?

Both have "cysts" in the medulla but Medullary Cystic has a poor prognosis, and medullary sponge kidney is benign with just a predisposion to kidney stones (and less often, hematuria and UTI's).

Given in the question "there are cysts" in the medulla, what are other major differentiating points? (If you remember that one UWorld question, you'll know why I'm asking).
