I hear my program doesn't like us doing moonlighting during our first year, is this a common trend around fellowships?

I think we're responsible enough to not push ourselves beyond what we can handle, I would like to have the option and pick up a shift once a month (or even 2 months) to buffer myself.

How do you even start looking for moonlighting opportunities? Do all "hiring hospitalist services" ask for letters from a PD if you're a fellow?

It has to do with work hours. Your moonlighting will count against your hours worked and your time/days off. That type of thing can get your program into really serious trouble. Many people do not moonlight 1st year, it's just too busy. By second year your clinical schedule will have cleared enough you'll have more time to work.
