I want to get a letter from an ER doc with whom I rotated during my internal medicine rotation. He is an ER attending at a hospital that is affiliated with my med school but not with the ER residency program. Would he be able to use the SLOR?

Where can I get this year's SLOR form? The one I printed out from www.saem is for 2002-2003?

You can definately use the 2002-2003 SLOR, because you are doing two years of clinical rotations, so its okay.

And I would go ahead and use the SLOR from that attending, as long as he's an EP you'll be ok.
