NDBE-1 application -- where do we get this from

I have requested Joint Commission in Chicago to send me an application for the NDBE-1 for the July examination and haven't heard from them.. more than 2 weeks.

Does anyone know if I can get this application online?

You can request an application form on-line. But you won't be able to apply on-line for Part 1. It has to be done the old fasioned way with pencil and paper. ADA takes their time with stuff. Call them (find their number on their website). You would find waiting for your results to be even worse experience! Imagine waiting 6 to 8 weeks! It's krazy!!
