Women in late 20s 3rd trimester pregnancy has right hemiparesis, a right hemisensory deficit in arm and face worse than lower extremities and language difficulty. CT results convinced she had L MCA stroke. No problem in CSF or cardiac workup. She had an initial moderate-severe deficit. She improved in walking, language comprehension improved somewhat from what was very poor initially. Her speech could be understood with difficulty by her close family. She was seen subsequently for minor gynecological issue, but no neuro eval was taken. Later on she lived on disability and apparently been a functioning parent with erratic tendencies - moody and childish behavior.

What are possible misdiagnoses over a L MCA stroke?

Definitely not in the format of a Step 1/2 question there so I don't know what you're playing at here. Lol, I read the first two sentences and was like, omg I can actually localize this but the answer was given in the very next sentence.

As for something that would mimic a stroke on CT that isn't a stroke? Not sure, are we talking ischemic or hemorrhagic (more likely seen with CT) first of all? Possible an STD with Neuro complications? Kluver-Bucy? I'm dead without answer choices lmao.

I might be reading this wrong but it seems like the question stem tells you that the dx is L MCA stroke. What it wants is possible diagnoses that are incorrect. Like a differential in the workup of the patient. I don't know why you're asking it in this forum though, there's an entire subforum devoted to USMLE questions.
