Need to find USUHS Form 1304!

Hey all,

As I'm filling out the paperwork for my ADTs, each location is asking me to fax over a completed USUHS Form 1304 before I arrive. Does anyone, mainly the USUHS students, know where I can find this form? It doesn't appear to be on MODS, an internet search has come up short, and I can't seem to find it on any USUHS webpages. Thanks for your help!!
