No BU supplementary forms?

Are there any forms for the BU secondary online that we need to fill out? Do we just type up the info they request in any format we wish and send it off? Thanks!

From what I remember, I just sent off a check and a photo. Thats surely an easy way to get $100 bucks for BU.
