No letter from my research supervisor

I've done a few projects with my current research supervisor. He was lined up to write my reference letter but I'm not so sure about that anymore. This past year, I fcked up a few times. I missed 2 deadlines and my presentation to the group wasn't very strong. Basically, he's a nice guy but he's not so happy with me at the moment. There is a chance that he will forget the hard work I've put into the projects purely based on recent events, so I'm no longer hoping for a strong letter from him.

How will it look to PDs to have these research projects but no letter of recommendation from the supervisor under whom I've done this research?

Nothing. They want 3 LOR from people who know you well. If you can have 3 from your core clinical rotations like IM, Surgery, Peds, ... You will be fine. If one of them is from a known radiologist, it is beter.
