Just like anything. Do a rotation with the person who utillizes this tool and make it clear you want to learn the skill. Maximize your time with the equipment and do enough to be proficient.

The Family Med faculty do our ultrasounds in the office and offer training to the residents. For the most part, all residents learn to do the essentials -- check for position, viability, AFI, and some measurements. If they want to learn more and do organ surveys, etc, they will go to a CME course and then come back and practice their new skills with their patients here longitudinally.
I doubt that any one rotation would give you enough experience to feel comfortable doing OB ultrasounds.
