I am chief
Perhaps GI is not for international graduates , I am really disappointment by the system
I am not sure whats the criteria for interviews ,
...I don't want to get into a huge discussion about this but have to make sure ur not fooling everyone else out there but CC, Mayo are not top 5, they are great programs with lots of clinical exposure. Hopkins is a top program but after being there for 4 years I know of many reasons why there are other programs that have a stronger clinical and research component. Also you see more international fellows at CC, Mayo, and even Hopkins due to their location in comparison to other top programs in better locations. From what I have been told from my faculty advisors it seems that a significant part of the decision process is based on clinical experience/abilities such as location of residency, medical school and board scores (used as a screen) and not just research. In the end unless you are applying for a specific research pathway we will all be spending far more time as clinicians than researchers. Research is certainly important but it is also something that most applicants have in one form or another so it seems like it is actually difficult to stand out in this way (unless you are running your own clinical trials or have a PhD or something like that). LOR are also important but as applicants we actually have no idea what they really say (assuming that we waved the right to view them which is a must). A generic letter from a "big name" that cannot comment on specific clinical skills or research abilities from a GI elective, floor month, research project etc will be much less helpful than a sincere letter from someone lesser known. Lastly, there are many international graduates at prestigious GI programs (Cleveland clinic, Mayo and Hopkins if you want to look at "top 5" programs) so I wouldn't say that the system fails international graduates.
