Outpatient musculoskeletal PM&R

If I complete a residency in PM&R, can I do only outpatient musculoskeletal rehabilitation (with the "necessary inpatient stuff") as a legitimate, lucrative practice. I am thinking about things like ortho rehab and sports med. Another option is working with an ortho group. Are these types of positions plentiful and do I need a fellowship to do something like that? Thanks for any input.
 
My friend who recently completed a PMR residency is doing strictly outpatient musculoskeletal stuff, but she is working at a combined ortho/pmr practice. She says that many pmr's do outpatient only, but obviously its more competitive since there's an easier call schedule ... (sorry advice is tangential, but no one else replied)
