how did you receive part 2 application form ?because, When I called they said once you pass part 1 we will send automatically.
any information will be greately appreciated

just call them & ask for the part-2 form ,tell them u r done with part-2.(they r not going to ask u anything more ).i got it this way b4 i got my result for the part-1.(& they havent send me a part-2 form with the score report!)

so just call them.
