There's a new program opening up in Florida, see http://ome.allkids.org... it obviously doesn't have any history, so applying there would likely be a good option.
Typically, those programs with stand-alone children's hospitals tend to be considered higher tier programs.
Roanoke Carilion and Fairfax INOVA I think aren't as competitive as other programs in Virginia. University of South Carolina program is another that comes to mind.
