Pharmacy licensure requirements for Illinois

Please forgive me for asking something like this on a forum, but I have had a very hard time finding clear, specific, up-to-date information on the requirements for pharmacy licensure in the state of Illinois online. I called the board directly with my question and for whatever reason, the man I spoke with did not know the answer to my question. This is related to a potential residency program I interviewed with in Illinois, I would like to figure out definitively if I should rank them (I tried to do my homework before I ever applied there but came up with very little information and decided to go for it anyway, although I was too embarrassed to ask this question during my interview).

What I would like to know specifically is if Illinois has requirements that a certain number of the 1500 internship hours come from certain sources?

There are some states I have looked at which only allow students to receive a certain number of intern hours provided through their SOP, with the remaining hours needing to come from an outside source (e.g. I suppose a retail or hospital internship).

I have seen some states that have this requirement for in-state-educated pharmacy students, but allow all 1500 hours of students educated out-of-state to come solely from their SOP.

If any of you can answer this for me, or point me to a clear, updated resource that can clarify this for me, I would appreciate it very much. I would be especially appreciative to have this question answered before 11:59 pm EDT on Friday, March 8th, since that is when our residency rank lists are due! All of my intern hours have come strictly through my SOP, so with this in mind I would like to make sure I am eligible for licensure in Illinois.

As far as I can tell, there isn't a specific requirement regarding how your internship hours are divided up.

Based on the Illinois application (http://www.idfpr.com/renewals/apply/forms/ph-ex-i.pdf), it seems like the ED-PHM form is the basis for whether your graduation requirements have been met. Note that item "M" doesn't ask for the breakdown of your internship hours.

You may have already called, but I found this contact information on Drake's website (http://www.drake.edu/cphs/handbookspolicies/pharmdstudenthandbook/):
The state of Illinois requires completion of 400 internship hours, all of which are provided in College of Pharmacy and Health Sciences curriculum. Questions regarding Illinois internship and licensure requirements should be directed to:

This was very helpful. I think the fact that all these resources I've looked at (and the ones you've shared here) are not explicitly mentioning that the hours must come from a certain place might ultimately be because there is no such requirement. That ED-PHM form has given me a little peace of mind as well.
