PHOTO FORM for Columbia

I think i must be going blind or something
cuz i cannot find the stupid photo form that columbia tells you to send to them

after i submitted my app on line
it said that whether u pay by cc or check,
you must send them a photography of yourself with the photo form

where do you find this form????

i cannot find it on the columbia website
or at the apply yourself website

Hmmm, I just mailed mine with something else I was sending them... I don't think there was a photo form unless I'm blind.

Don't think there is a form at all. If there is actually one, we got three blind people here

just turned in my app yesterday and didnt see any photo form. i am just going to mail them a photo tomorrow

ps - do we only have to mail them a photo if we paid by credit card or did they want a reciept of the payment too? i cant remember. thanks
