I want to put down some of my leisure hobbies like playing trumpet and swimming, but since i didn't take any swimming or music class, I don't know whether I should still put them on my application.
Any comment is appreciated!

Why not list the Humane Society activities under community service instead of academic recognition?

You can list hobbies under that designation. You don't need to be formally trained or have taken a class in it.

Your extracurriculars look fine to me. You have something to list for the those categories usually considered the most important.
 
The only concern is that I only went to the Humane Society once... is that ok?
