psychology issues with men

i have a friend who is struggling with some psychological issues, and has been for years. he is in therapy. he seems to be bipolar but is not medicated. problem is, he won't tell me what it is that's scarring him, and it's becoming a barrier in our relationship. i want to help. i've been researching, trying to figure out what can scar a man so deeply that at 45 he still feels like he can't open up about it, at least not to me. i've known him a while. is there anyone with any insight? my closest guess at this point is child sexual abuse but he never says anything about having an unhappy childhood, so i'm at a loss.
