For fourth year what I've been doing, and it seems to work for the 2 aways I've done so far, is to just say that you want a letter in the beginning and to give you feedback throughout your rotation. I did get lucky and have both my attendings stay with throughout my time as a sub-I but I know not everyone has that luxury. I was told that if you're up front at the beginning that you need letters and let them know ahead of time that most attendings will work with you assuming you're a good fourth year. YMMV but I think the first and best step is to talk to the attending in the beginning and let them know that you hope to get a letter from them in the future and good feedback about your progress.
