if you really want those "top" programs, i would imagine that you have nothing to lose in ranking them high.

it will lessen your chances at #3 or #4 only if you feel you have a solid chance at matching the top choices right?

are you a *shoe in* at the small community program near your house??? if that is where you wanna go, then you might as well rank it #1 and end ALL this madness right......

If you think you may want to do derm, do not go to the small program. Go to a program with a derm fellowship and a strong derm attending. You need to do some derm research and get them to network you to all the other derm gurus around the country.

If you plan to do private practice as a general pathologist in the local area then by all means go to the smaller program. Realize you may have to do alot more self directed learning than in a bigger program.
