I have almost all of my application done and all of my other recommendations done, and I might be able to get a recommendation from a doctor that I job shadowed, who is the chairman of medicine at a hospital. But, we just started to meet up with each other, and I don't know how to ask him for it without coming off a jerk.

I wouldn't bother asking him if you already have enough letters.
