how does the radiology residency work - is it 4 years and then fellowship is optional or is fellowship mandatory?

Internship + 4 year residency.

Some programs that aim to train academic radiologists (e.g. Hopkins) require a fellowship, but most programs do not. Getting a fellowship is pretty easy now, since the private job market is so hot.
