Residency in nordic countries or Germany for portuguese doctor

Hello everyone!
I'm a portuguese doctor currently starting orthopaedics residency, but it's my wish to change specialty and move abroad.
Nordic countries are my preference, particularly Sweden, but it's absolutly impossible to learn the language in Portugal and I can't afford to risk my current job and move to another country just to learn the language. Therefore I ask for your help... can you tell me if the AT position in Sweden is paid? and is language proficiency required for applying to an AT job? And what about the ST position, is language proficiency required? Do you have any idea if it's hard to get into ORL dermatology or Ophtalmology?

German is much easier for me to learn, so I'm considering gGermany as well.. But I know their economy is in great recession, so I'm not sure it's such a good idea to go there... Can you help me? And do you know if it's hard to get a job in ORL, dermato or Ophtalmo?
