Retention loan vs educational loan repayment

I am searching for a job. I am an IMG so fortunately no education loan.Is there any advantage to a private non profit organisation for offering education loan repayment? Do they get money from federal/state government or tax advantage?
I can understand the desire to get US medical graduates for an organisation, but otherwise from financial perspective can they give a retention loan.

I am guessing that if sign on bonus is partly taxed every year and forgiven, then it is retention loan. if it is paid and taxed in one year then it is bonus.

Besides negotiation, I am also curious that loan repayment is a bigger amount like 100000. I understand that it is helpful to US MD's and rightfully so.

But what is the financial benefit to a non profit hospital to offer 100k as loan repayment but only 20k as bonus if no educational loan is there.
