If I get into my state school, would it be possible to do all of my 4th year rotations at a hospital in the midwest? I want to do residency and practice in the midwest and I can't wait to get there, but I know I have a better chance of getting into my state school down south.

all the schools that i have been to put a cap on the amount of away rotations you can do. i am pretty sure this is common practice and from what i know, on average, you can do a max of 3 away.

So I guess it's just osteopathic schools that let you do this?

Yeah, MD schools have a limit on "away" rotations. I don't know about DO schools. But you can definitely get to the Midwest by doing a few rotations out there and then applying primarily to residencies out there.
