If one were to have a service loan in the midwest and serve in a semi-rural to rural community and after his/her commitment is up ( 4yrs) would it be difficult to go to one of the coasts and get a gig in an urban ED?

I assume you mean you have a public health loan which requires that you practice in a particular area for 4 years. After that, you're a free agent, just like everybody else. Your chances are as good as most when you're moving.
