All I can see to improve on is primary care shadowing. If you wanted to gild the lily, you could take another gap year - but I do not think this is needed. Your ECs are strong and your application stellar. You also have a good adversity/diversity story. Aim high: 10 top 20s, your state schools, and five midtiers. Good luck!
