If someone went to medical school in Puerto Rico, like San Juan, UPR, Central University, or Ponce, are they considered foreign graduates like the ones who go to the Caribbean med schools? Or, are they considered American medical students like those who graduated from the main 50 states?

Also, how ARE Puerto Rican Medical Schools? I have heard that Ponce is very good, but thats all i know.

No, they are US medical schools and are treated as such by residencies. A few threads have discussed this in greater detail; try searching PR med schools, puerto rico, etc. using the forum search function. Varying degrees of Spanish ability are a necessity depending on the school, and UPR is a strict state school, like, say, UMass (rare to have an OOS acceptance without strong ties to the island).

Puerto Rican med schools are accredited by the same organization that makes sure Hawaii and Mainland US med schools are up to a set standard. Their grads are treated like US seniors in the residency matching process. While classroom instruction and texts may be in English, Spanish proficiency is expected since the clinical years require interaction with Spanish-speaking patients. The MSAR will tell you what "ties" are expected to get serious consideration from them.
