In my first response, I assumed the OP was asking which schools start actual clinical rotations before the MSIII year. If you are just asking about clinical exposure/preceptorships, etc., I think the majority of med schools do start those in the first year.
Rochester currently has the earliest real clinical exposure I've heard of- clinical training starts first semester and then you do your first ambulatory rotation the second semester of first year.
