So,

My class rank came in for the first year and I'm at the top of bottom quartile. A few things to note:

My GPA is a 3.0. I'm not barely passing classes. Our averages are pretty high - which makes a B average in the bottom quartile.
My school didn't count the spring quarter in our rank, which is where all my better grades were. I struggled a LOT in the beginning of med school and was able to get it together in the spring quarter.

Should I be worried? I think if my spring grades were included in the rank my rank would be above the bottom quartile, but who knows.

No

What would being worried accomplish anyway?

Nothing. I guess my question is - will this hinder my abilities to do the residency I want/will this have long term repercussions? I am interested in FM, Psych and possibly Neurology.

No. Takes some people the first semester to adjust (myself included). If you did better in the spring semester and not having trouble passing classes, just keep chugging along and try to improve yourself and understanding of medicine. Rank will improve with self improvement, even if it doesn't, your choice of residencies won't be weighting your rank heavily (For FM, Psych)

Your preclinical GPA is one of the best indicators of your Board scores. That said, it sounds like you have a rising GPA trend, so I'd go off of how you've done your performance in OMSII, not your total GPA. Your class rank is apparently skewed down by your early poor performance.

As such, I think that you will be fine!!!

Preclinical grades are of little interest to PDs. Step I/COMLEX I...that's what counts.

Don't worry so much about this right now, I think maybe focus on doing well during your second year and fully learn/master that material. It will help a lot for boards. Good luck!
