Should I consider some DO/Caribbean schools too?

My letters of rec, personal statement and resume are all pretty good, but not really anything extra special. I don't, for example, have a shining LOR from a doctor, a summer volunteer experience in Africa, or any published research articles. Just a committee letter, clinical job, and good grades.

I feel like I am just good enough to miss the cut at most MD schools.

With broad and wise application to MD schools, you should probably be able to get an acceptance. I think it'd be prudent to include a few DO schools you might like to attend, also. Definitely do not bother with the Caribbean.
