I'm just beginning third year and foresee having to ask attendings for letters of recommendation. I was curious if ERAS has a standardized form that I can just hand to him/her and have it filled out and kept on file at my school?

Not unless things have changed from previous years. The provide you with a waiver form to sign and attach to the letters but IMHO most US faculty would be familiar with the standard format for an LOR.
