what if you dont know what you want to do.

a lot of ple know what they want to do bc of their Great expereinces, but i have just done more research and not as much clinical as i would have liked, so how do i know what i want to do and how can i tell stanford that.

Perhaps private practice? I mean, I think it's ridiculous they don't have an option for primary care, but I guess private practice is the closest they come.
 
Surgeons do sometimes do research. I don't know what type of research you're talking about; it's pretty difficult to balance basic research and surgery as they are both demanding careers that have nothing to do with each other on a day to day basis.

My uninformed guess is that if you wanted to do burn surgery primarily or solely, you would need to be at a lrage academic hospital to have enough cases.

Why burn surgery? Do you have some basis for thinking you want to do that in particular?

They aren't asking about your specialty, they are asking about practice setting, which is mostly orthogonal to what field you're in. Plenty of doctors do primary care in academia, and there are many subspecialists in private practice.

Yes, I realize they weren't asking about specialty. They asked, verbatim, about career goals. "What knowledge, skills and attitudes have you developed to prepare you for this career path?" if I remember correctly. I do think they're asking, at least partially, about career plans, and I don't think surgery qualifies as academic medicine.
