Stanford Secondary "Waiver Form"

I'm a little confused about the "Waiver Form". It kind of looks like the forms you give to people who write you Letters of Rec's.

Do we have to fill this out? If so, all we do is check our preference and sign it?

if you have a pre-med advisor or committee system which has their own waiver forms..
then, you don't have to fill them out.
at least that's what i got from reading over it quickly.
wow! i can't believe i got past the primaries.. i kinda sent it in to stanford as a real reach! =)
